激変した環境に適応するための発想と行動へ
Toward New Thinking and Action to Adapt to a Rapidly Changing Landscape

　３年半に及ぶ新型コロナウイルスとの闘いで、患者の皆さんも鍼灸師も経験したことのない多くの犠牲を払い、ポストコロナの新しい時代に向けて希望の光を見いだしながら、耐え抜いてきた昨今です。

　こうした大きな時代の変化の中で、日本鍼灸師会の会長という極めて重要な職責を担わせていただくことになり、改めて責任の重さを実感しております。

　今後の日本社会ではり師・きゅう師の役割は今まで以上に嘱望されるものと思われます。そのためには日本鍼灸師会のこれまでの目標とともに、鍼灸の普及によって国民に鍼灸師が広く認知されるよう活動し、鍼灸師が働き甲斐ある社会づくりをすることに組織活動の重点を置くことが必要であろうと思います。

　超高齢化社会と労働力の不足が深刻化する２０４０問題では、社会保障制度の崩壊危機が予想され、給付と負担の見直しも予測されています。私たち鍼灸師が社会の中で果たすべき役割として、若人の健康維持増進と高齢者の健康寿命延伸があります。

　その役割を全うするためには、医療従事者の方々に、会員はもとより全ての鍼灸師について、理解と協力得ることと、国をはじめとする関係機関と私たちの組織が連携することが不可欠です。

　そこで今期はこれまで以上に対外活動に重きを置き、鍼灸学術専門団体や鍼灸の教育機関との結びつきをより強固にし、知見に基づいた鍼灸施術が国民に周知され、鍼灸施術の受療率が増えていくように積極的に活動を進めていきたいと考えています。

　その具体的な活動に関連することとして、令和６年度の社会保障費改定があり、医療、介護、障害のトリプル改定が行われます。次回の改定は前述しました2040年問題を見据えた改定でもあり、社会保障制度は大きく影響を受けます。

　現在の日本の社会保障制度は、高齢化社会への対応を目指して構築されていますが、2040年問題を鑑みると、現行の医療保険制度の中で高齢者の医療費は大きく増加すると予想され、また、社会保障の持続的運用を鑑みると、医療制度の改革や財源の見直しが図られると思いますそして医療や介護の包括的対策で経費の削減を求められ、私たちが取り扱うあはき療養費も大きく影響を受けると思われます。社会保障制度の持続可能性を確保するためには財源の確保が欠かせませんが、同時に支出の見直しや効率的な予算配分など、財政の健全化を図ることとなります。

　私たち鍼灸師は低コストで利用者満足度を高めるサービスを提供できますが、財政の健全化という目線で考えると、サービスに見合った効果を上げることができなければ、あはき療養費制度の継続は危ぶまれます。ただ漫然と施術をおこなうことなく、「治す」あはき施術が求められます。日本鍼灸師会は、社会に求められる鍼灸師育成を通して鍼灸師を守らなければなりません。全国各鍼灸師会と連携し、日本鍼灸師会の会員数増により、役員の保険者へのアプローチや利用者への周知活動を行う活動費が増やせます。

　この度の役員改選で、理事会は前例のない新人理事が多数の構成となりました。目まぐるしく変化する日本で、時代の変化に上手く対応すべく既存事業の見直しを行い、短期的中長期的目的について理事一人一人から声を聴きながら想像し、目的達成に向けた合理性をもった組織づくりを考えています。また、全国の代議員の皆さんから承認された再任理事の経験に加え、新任理事が持つ感性にも大いに期待したいと思い、各委員会を担って行って頂く事と致しました。

　今後ますます変化する社会に求められる鍼灸師像を想像するだけでなく、現実化させていきます。これから２年間、各理事は自身の思いを書いた立候補所信表明の内容を基に事業に取り組まれることと思います。意思決定の場の理事会では、、横断的に多くの意見を出し合って議論する場として、決定した事項に対してはこれを成し遂げるよう同じ方向を向かなければ進展はありません。役員全員が一丸となり会務を進めて参ります。

　日本鍼灸会はこれからも全国の４８鍼灸師会と協力し、誰からも信頼を寄せていただける組織となるよう努めるとともに、医療を取り巻く環境が大きく変化してゆく中でも中心軸が揺らぐことがないように、国民の健康維持の増進を優先目標に据えて活動していきたいと考えています。

　皆様のご意見に耳を傾けつつ時代に適応した活動を進めたいと思いますので、ご支援ご協力をいただきますようお願い申し上げます。

After more than three and a half years of confronting COVID-19, the world has endured tremendous sacrifices while gradually finding hope for a new post-pandemic era. Even during this challenging period, we continuously asked ourselves what contributions acupuncture practitioners could make for the benefit of the public.

Since the easing of the pandemic, we have focused our organizational efforts on promoting wider public recognition of acupuncture through its dissemination, while also working to create a society in which acupuncture practitioners can find professional fulfillment.

Looking ahead to the so-called “2040 problem,” in which Japan’s super-aging population and labor shortages are expected to intensify, there is concern about the sustainability of the social security system, including potential revisions to both benefits and public burden. Within this context, we believe that the role of acupuncture practitioners includes supporting the health and well-being of younger generations and extending healthy life expectancy among older adults. To fulfill this responsibility, it is essential to gain the understanding and cooperation of other healthcare professionals not only for our members but for all practitioners and to strengthen collaboration between our organization and relevant governmental and institutional bodies.

Accordingly, during this term we intend to place even greater emphasis on external engagement. We will further strengthen our relationships with academic organizations specializing in acupuncture and with educational institutions, and we will actively promote public awareness of evidence-informed acupuncture practice so that utilization of acupuncture services continues to increase.

Japan’s current social security system has been designed to address the needs of an aging society; however, in light of the anticipated demographic changes by 2040, medical expenditures for older adults are expected to rise significantly. At the same time, efforts toward sustainable social security will likely involve reforms to the healthcare system and revisions to funding structures. Comprehensive measures in healthcare and long-term care will increasingly demand cost containment, which may also have a substantial impact on the reimbursement system for acupuncture, moxibustion, massage, and related therapies.

While acupuncture practitioners can provide services that achieve high patient satisfaction at relatively low cost, the continuation of reimbursement systems will depend on our ability to demonstrate outcomes that justify those services. It is no longer sufficient to provide treatment without clear therapeutic value; there is an increasing expectation for interventions that achieve measurable improvement. The Japan Acupuncture and Moxibustion Association must therefore protect the profession by fostering practitioners who meet societal needs.

By working in cooperation with prefectural associations nationwide and increasing membership, we can expand the resources necessary for outreach to insurers and for public awareness initiatives.

In a rapidly changing society, we are reviewing existing programs and, while listening to the perspectives of each board member, envisioning both short- and mid- to long-term objectives in order to build an organization that operates with clear purpose and rational structure. In addition to the experience of reappointed board members, we also place great value on the fresh perspectives of newly appointed members, entrusting them with leadership roles across various committees.

To realize not merely imagine the future role expected of acupuncture practitioners, the Board of Directors will promote open and cross-departmental discussion that transcends hierarchy. Once decisions are made, we believe progress depends on all members working together toward shared goals.

The Japan Acupuncture Association will continue to cooperate with acupuncture associations throughout the country, striving to remain an organization trusted by all stakeholders. Even as the healthcare environment undergoes major change, our core mission will remain unwavering: prioritizing the promotion and maintenance of the public’s health.

We will continue to listen carefully to the opinions of our members and stakeholders as we advance activities that respond to the needs of the times. We sincerely request your continued support and cooperation.
